I don't understand when they say bind off 5 stitch at beginning of row and then again it say  bind off 5 stitches at beginning of row. Do they mean on each side.

Yes, they mean 5 sts at one side, take the carriage across, then bind-off 5 sts at the other side.  It has to be done this way because you can only bind-off on the side that the yarn (thus, carriage) is on.  A decrease of 1 or maybe 2 sts could be done at both sides of garment at the same time without having to move the carriage a row between the decreases, but when you get to binding-off 5 sts at a time, you have to use the yarn from the carriage to accomplish it.
