I haven't seen this before, but reading the instructions I think I can answer your questions.

You do not cast on in rib - unless you want a rib section below the main part of the work.  The main part is knit in stockinet until you get to where you want to work this pattern.  Then you push your ribber up into working position, pitch H and push up to WP all the ribber needles that fall between the 2 end needles on the main bed.  Being pitch H, the ribber needles will be between the main bed needles, not directly opposite them..

Step 4, push all the ribber needles up to UWP and then push them down to NWP so that the stitches all drop off the needles.  Then simply lower the ribber to its "resting position"  and carry on.

You don't ever need to remove the ribber, even if you aren't using it.  I haven't taken my ribber off my main bed for years!

Thanks Jenny for the clarification. It appears that you are just using the ribber needles to enlarge the stitches ... right?
